The Physical Therapist evaluates and assesses the patient’s current functional level.  From the evaluation, a program is designed and implemented to assist the patient to achieve optimal functional level.  This is completed within the guidelines of the referring physician.  

  • Assist the physician in evaluating the level of function, disability, injury, or disease process of individual patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests appropriate to the age of the patient.
  • Interpret, plan, establish, and exercise proper treatment program in accordance with the physician’s referral, periodically reviewing the patient’s program and response and making recommendations to the physician relative to the need of continuing or discontinuing the physical therapy.
  • Evaluate the home environment and make appropriate recommendations.
  • Provide treatment as ordered by the attending physician to relieve pain, develop or restore function and maintain maximum performance, using physical means such as exercise, massage, heat, water, light and electricity.
  • Observe, record, and report to the physician the patient’s reaction to the therapy program or any changes in the patient’s condition.
  • Work in collaboration with the physicians and other members of the health professional team in determining the needs of the patients and developing their total plan of care.
  • Contact the physician directly for patient reports and care as needed.
  • Maintain a level of professionalism at all times and report unethical situations to the Physical Therapy Board or appropriate personnel.
  • Participate in all mandatory case conferences
  • Instruct, teach, and supervise patients, their families, and other health team personnel regarding physical therapy procedures as related to the patient’s therapy programs.
  • Participate in staff development activities, which may include providing inservice training to other home health personnel; participate in inservice education programs.
  • Supervise PTA(s) and adheres to all rules, regulations, code of ethics, guidelines and codes in federal, state and local laws which are concerned with the practice of physical therapy.
  • Treat and reassess patients seen by the PTA every 6th visit but not less than a month.
  • Conduct face to face conferences with each PTA to review progress and modification of treatment programs for all patients seen by the PTA
  • Participate in Performance Improvement activities 
Position Requirements
  • Licensed Physical Therapist in the state of practice
  • Possess knowledge of Medicare and Medicaid guidelines essential to Home Health Coverage
  • Demonstrate the ability to assess the interpretation and formulation of patient care plans to meet the needs appropriate to the age of the patients serviced by the Agency
  • Demonstrate proficiency in clinical documentation
  • Valid drivers license, and reliable transportation
  • Computer, internet access and email capabilities from home or other off-site location


  • Graduate of an approved professional program in Physical Therapy.

Training and Experience

  • One year therapy experience with preferably one year experience in the field of Home Health Physical Therapy

Physical Requirements

  • Work is performed in patient homes, the employee’s vehicle, and in an office environment.
  • Work requires continual mobility.
  • Frequently sits, stands, stoops, twists, turns, reaches, and grasps.  Able to stand or sit for prolonged periods of time.
  • Continually communicates in person and over telephone systems with patients, staff, and other health care professionals; this requires good hearing and speaking skills.
  • Close-up vision is frequently required.
  • Frequently exposed to the hazards of city driving and is often exposed to temperature extremes.
  • Sometimes exposed to potentially unsafe and unsanitary conditions in patient homes.
  • Frequently exposed to biohazards in performing patient care activities.
  • Frequently pushes and pulls a typical weight of one hundred (100) pounds; maximum push/pull weight is two hundred fifty (250) pounds.
  • Frequently lifts and carries a typical weight of ten (10) pounds; maximum lift/carry weight is fifty (50) pounds.
