Registered Nurse (RN) Care Manager - Care Partners (Facility Based Care) Centerville/Kettering 


The hospice care manager on the Care Partners team is a registered nurse (RN) who is responsible for overseeing the care of hospice patients residing in nursing homes, extended care, independent living and assisted living facilities. The hospice care manager uses keen assessment skills to recognize pain and symptoms and collaborates with the hospice physician to keep the patient comfortable and pain-free. A strong clinical background is essential because the work is very autonomous and involves caring for patients with various disease processes, so a broad base of knowledge such as that developed in a Medical-Surgical setting or as a Float nurse provides good preparation for this work.

The hospice care manager educates families, partners with facility staff and other caregivers on recognizing symptoms and providing safe and competent care for the patient while offering emotional and practical support for both the patient and their family/caregivers. The care manager must have excellent communication and time management skills, be comfortable around dying patients and be able to educate patients and families on the signs, symptoms and expectations of the dying process. RN Care Managers must be compassionate and patient and must respect the unique differences of their patients, caregivers and facility staff.

The hospice care manager works as a part of an inter-disciplinary team consisting of Chaplains, Social Workers, Home Health Aides, Volunteers and Physicians who meet bi-weekly to coordinate physical, emotional, and spiritual care of the patient and family. They supervise the care provided by Home Health Aides (HHA) and Licensed Practical Nurses (LPN's). The Care Partners care manager will also attend Care Coordination meetings with facility staff.

This is a full-time salaried position, primarily Monday through Friday from 8 – 5. The Care Manager is assigned to a territory within our 8 county service area, but must be flexible to go where needed.

Registered Nurse (RN)-State of Ohio
CPR Certification
Certified Hospice and Palliative Nurse (CHPN) preferred
Active Drivers License
Valid Automobile Insurance

** A Safe driving record is mandatory: no more than 3 moving violations or more than one chargeable accident in the past 36 months and no major convictions (drunk, drug or reckless driving) within the past 7 years.
