Description

The purpose of the Nurturing Families Network is to identify and make contact with first-time mothers receiving prenatal care at the Norwalk Community Health Center, delivering at the Norwalk Hospital or receiving services elsewhere in the community and link these families to appropriate community resources.  Support service to include enrollment in the Nurturing Families Home Visiting component for those families identified as being at risk of abuse and neglect.

 

The Home Visitor is responsible for initiating, contracting and maintaining a relationship with NFN families. This includes a regular schedule of weekly home visits of at least one hour, for up to a five year period. The responsibilities include but are not limited to:

 

  • Meet regularly with families who have agreed to Nurturing Home Visits.
  • Complete all necessary and required paper work for the program
  • Maintain a caseload of 15-20 families
  • Help clients connect with appropriate health and social services
  • Accompany clients to medical and agency appointments as necessary
  • Serve as an advocate for clients with health care and agency providers
 

Position Requirements

  • Knowledge and expertise in the areas of outreach, prenatal/post partum health care, diverse¬ ¬ populations, child development, parenting and community resources, and/or minimum.

  • Associate's degree in human services, social work, public health education or related discipline.

  • Bi-lingual is required

  • Must hold valid Connecticut drivers license & have reliable transportation

  • Must have awareness of cultural and socioeconomic characteristics of clients served.¬
