Community Home Supervisor - Days/Evenings - Monessen, PA 

Description

The hours for this position are 7:00am-3:00pm/ 3:00pm-11:00 split shift, 6 days on/2 days off.

Responsibilities include:

  •  Perform the essential functions of the Residential Companion.
  •  Promote and follow safe working practices and take the necessary actions to prevent injuries and illnesses to staff, clients, visitors, and volunteers.
  • Conduct and/or coordinate ongoing training and guidance of all respective house staff.
  • Coordinate with Director, Community Homes; Senior Supervisor, Community Homes; Program Specialist; and/or Community Homes Nurse to provide house orientation and on-the-job training for new hires.
  • Monitor and evaluate staff performance, including, but not limited to, performance evaluations, counseling, and disciplinary actions.
  • Assist in interviewing applicants and make recommendations on new hires to the Director, Community Homes and/or Senior Supervisor, Community Homes.
  • Responsible for staff scheduling, including, but not limited to, work assignments/ rotations, staff training, staff vacations, overtime assignments, and back-up for absent staff.
  • Ensure appropriate coverage in the event of a staff vacancy by either assigning qualified staff or, if applicable, must be able to perform essential functions of staff positions.
  • Establish an individualized professional growth plan for each staff, in conjunction with staff’s input; conduct staff performance reviews; and follow-up on staff progress.
  • Supervise and mentor staff and ensure staff compliance in accordance with all legal, regulatory, and licensing requirements and Agency policies, procedures, and rules.
  • Perform first aid and CPR as certified.
  • Evaluate information to determine compliance and comply with standards; 6400 regulations; Agency policies, procedures, and rules; and all other applicable regulations.
  • Review and work with the Director, Community Homes; Senior Supervisor, Community Homes; Program Specialist; and/or Community Homes Nurse on responsibilities regarding the Community Home, programming, binders, finances, etc. to ensure compliance with county and licensing requirements and Agency policies and procedures.
  • Ensure the Community Homes are in good condition and operating in accordance with fire and safety rules and regulations.
  • Report all incidents involving clients to the designated Agency Point Person by following Home & Community Services Information System (HCSIS) guidelines.
  • Oversee client health appointments, annually and as needed, for medical, dental, vision, auditory, psychiatric care, etc., to ensure compliance with licensing regulations.
  • Monitor and evaluate expenses of the assigned Community Home to ensure the program is run efficiently as well as economically. Review the budget and maintain line items within budgetary allocations from general ledger.
  • Act as the primary liaison with operational and administrative departments within the Agency, as well as with interdisciplinary team members, other agencies, families of clients, etc.
  • Coordinate and participate in an on-call rotation with the Director, Community Homes and/or Senior Supervisor, Community Homes.
  • Monitor and address compliance with standards by performing random, unannounced Community Home visits outside of routinely scheduled working hours in coordination with the Director, Community Homes and/or Senior Supervisor, Community Homes.
  • Review, complete, and/or authorize all required Personnel forms/paper work (i.e., Time Sheets, Expense Sheets) and submit in a timely manner.
  • Review, address discrepancies, and sign Medication Administration Records monthly.
  • Review Human Resources training/supervisor reports and follow-up as needed.
  • Respond appropriately to the needs of the assigned partner house as needed.
  • Hold staff accountable by implementing disciplinary action, including suspension, when warranted.
  • Report to work if an emergency situation should arise, if required by the Director, Community Homes and/or Senior Supervisor, Community Homes.
  • Schedule and conduct staff meetings at least every other month.
  • Comply with responsibilities outlined in Lifesteps’ Emergency Preparedness and Universal Precaution Plans.
  • Travel on business as required.
  • Comply with confidentiality policies and procedures, including, but not limited to, the Health Insurance and Portability and Accountability Act (HIPAA).
 
Position Requirements

quirements include:

  • A minimum of six months experience working under 6400, 6500, and/or 2380 regulations
  • Bachelor’s Degree and 1 year experience working directly with persons with disabilities; or Associate Degree and 2 years experience working directly with persons with disabilities; or 5 years experience working directly with persons with disabilities.
  • Possess a valid driver's license which legally permits operating a motor vehicle in Pennsylvania.
  • Supervisory experience preferred.
  • Coach and provide career development to staff.
  • Complete and submit in a timely manner basic administrative paperwork.
  • Conduct employee disciplinary action as required in coordination with Human Resources.
  • Conduct interviews, recommend new hires, and provide employee program/department orientation.
  • Coordinate activities and special projects to ensure quality and meet timetables.
  • Coordinate with Supervisor/Department Head and Human Resources for appropriate staffing levels.
  • Develop and maintain a cohesive work team.
  • Develop, analyze, and prepare reports.
  • Evaluate department processes. Recommend to supervisor needed changes based on process analysis.
  • Guide, direct, and motivate subordinates.
  • Drive a vehicle without accidents or violations.
  • Lift, carry, push and/or pull up to 200 pounds with assistance.
  • Lift, carry, push and/or pull up to 50 pounds without assistance.
  • Respond calmly in emergency situations.
  • Tolerate a high volume of noise from multiple sources (talking, crying, yelling, etc.).
  • Work under pressure.
  • Work with aggressive, violent and diverse behaviors.
