Adult Inpatient Registered Nurses (RN) 


Bridges Evaluation & Treatment Facility is a sixteen bed facility that serves adults who require inpatient treatment due to an emotional crisis or change in function due to a mental illness. The program provides evaluation, stabilization, and brief treatment for men and women age 18 and older. An integrated, multi-disciplinary team of professionals provides individualized treatment and coordinates a comprehensive discharge plan that includes family and emphasizes utilizing community resources wherever possible.

(2 Positions) Schedule: 1 position - On-call, as needed basis. 1 position - 16 hours a week

Responsibilities include: providing direct nursing services within the standards of nursing conduct or practice of Washington State. Contribute nursing knowledge and skills to the rapid evaluation, treatment and subsequent stabilization of adults admitted to acute evaluation and treatment facility. This includes, but is not limited to:


  • Demonstrates comprehensive understanding of the concepts and principles that drive effective trauma-informed treatment by consistently:
  • Adheres to applicable regulations and Comprehensive’s policy/procedures regarding use and application of seclusion and/or restraint.
  • Applies QPRT (Question, Persuade, Refer, Treat) principles in assessing risk of suicide and collaborates with client, client’s family/friends, and co-workers to develop, implement and document an effective individualized safety plan.
  • Actively engages in team meetings, treatment conferences, and other opportunities to share/gain knowledge and support continued development of safe, effective, and trauma-informed client centered services.
  • Utilizes a variety of resources to increase clinical/professional competence.
  • Collaborates with Division Director, co-workers, and (as indicated) clients/family/community members to resolve potential differences of opinion/conflicts that impact provision and/or outcome of services
  • Has consistent and punctual/reliable attendance as scheduled.


  • Performs an Initial Nursing Assessment.
  • Monitors clients for signs and symptoms of acute illness, change in chronic medical condition(s), and/or other change in condition.
  • Monitors client behavior and communication (verbal and non-verbal) as requested. Provides immediate interventions as indicated and notifies client’s therapist.
  • Trains and supervises those to whom nursing tasks (e.g.: vital signs, blood draw) have been delegated to ensure consistency of service provision and accuracy of report.
  • Administers medication as ordered, monitors for effects/side effects.
  • Reconciles client’s medication list at time of admission and discharge.
  • Provides education regarding medication, health conditions, nutrition/health habits, and other applicable areas within structured treatment groups and individually to clients and/or their families.
  • Evaluates and implements appropriate treatment of minor medical conditions including, but not limited to, superficial laceration or abrasion, sore throat, skin irritation, routine wound irrigation/dressing or simple pain.
  • Refers complex conditions to medical provider in a timely manner.
  • Facilitates rapid transport to local emergency department in the event of a medical emergency.
  • Completes scope of work within scheduled work hours.


  • Confers with previous/current providers regarding client’s health status. Documents collateral and/or consultative contacts regarding the client, client’s treatment, and anticipated discharge in the medical record according to Comprehensive’s protocols.
  • Communicates findings of initial and ongoing assessment(s) to internal/external providers and others whom client identifies as needing to have the information in a manner that the recipient understands.
  • Communicates information in a timely manner using concise, respectful, culturally sensitive, person-centered verbal, written (paper and/or electronic), sign or alternative language that is understood by recipient(s).
  • Documents initial Nursing Evaluation, any changes to the evaluation as a result of information obtained after completion of the initial evaluation, and the potential impact of that information on the anticipated post-discharge financial and/or on the intervention plan.
  • Contributes to team identification and documentation of a) client’s concerns or needs which may impact the client but will NOT be the focus of treatment during the client’s inpatient admission, b) plan for referral to/communication with outpatient providers for follow-up, and c) actions taken.
  • Consults with treatment team members prior to making decisions that are outside the agreed upon protocols of the facility.
  • Documents direct nursing service, collateral contacts, recommendations for follow-up and other information in a manner consistent with Comprehensive’s policy and guidelines for method, means, content, format and timeliness.
  • Maintains accurate medication administration record for each client.
  • Maintains accurate medication and medical supply inventory and usage logs.
  • Consistently communicates therapeutic boundaries/parameters of role and relationship to client and members of the client’s family/support network.
  • Provides written and/or verbal testimony to the Court when requested.
  • Completes incident reports in a timely manner.


  • Contributes nursing conceptualization of client strengths/challenges to the collaborative process between client and treatment team through which the target areas which will be the focus of inpatient treatment are identified. Integrates nursing interventions into the client’s Interdisciplinary Treatment Plan.
  • Contributes to team identification and documentation of client/family concerns or needs which may impact the client but will NOT be the focus of treatment during the client’s inpatient admission and makes recommendations for follow-up.
  • Communicates nursing observations to other members of the treatment team and makes recommendations for treatment plan modifications in a timely manner.


  • Contributes to the treatment environment by maintaining temperature, lighting, ambient sound, appearance, scent, furnishings, activity level, flow of people and other factors in a manner which enables clients to actively engage in treatment and recovery.
  • Adheres to all of Comprehensive’s guidelines and polices pertaining to safety including, but not limited to, infection control, fire prevention/suppression, emergency response/management, facility security, injury prevention and response, and other aspects of environmental support associated with assigned duties.
  • Assists co-workers with the management and provision of meals/food, containment and removal of garbage, and provision of clean/undamaged linens and supplies in accordance with Washington State Department of Health standards, dietary orders/plan and Comprehensive guidelines and policies.
  • Maintains the facility and grounds in a neat and clean state at all times.



Position Requirements

Education/Experience: As required to obtain professional licenses and certifications.

Credentials: Washington State RN License

Other Credentials Required: First Aid/CPR Certification, Driver's License, Vehicle Liability Insurance
