ProMedica Home Health Care and Hospice, formerly Heartland Home Health Care and Hospice, is currently searching for a full-time, salaried at 32 hour/week, hospice and home health Triage Registered Nurse. As a triage nurse you will be responsible for handling incoming calls after hours, doing clinical assessments over the phone, providing clinical expertise and coordinating visits with the on-call staff for select agencies as needed/appropriate.
This is a position working from home and has a schedule that includes working every Friday, Saturday and Sunday, 4:30pm-8am. Must also be able to work any day of the week and weekends anytime between 8am-8am Saturday and Sunday depending on need. Currently 2 holidays/year 8am-5pm or 5pm-8am (shift times subject to change) required. This role will also be a backup for call offs and could back up 2-3 days/month.
This position will triage for several home health and hospice locations within WI, MN, IN, IA, and IL. May consider experienced candidates with only a WI, MN or IL license to start with the understanding that they will be required at their own expense to obtain remaining licenses shortly after starting employment with ProMedica.
The Triage Nurse is responsible to provide nursing triage services. Functions to assess the severity of the patient's health concerns, implementing nursing interventions and arranges skilled nursing or support staff visits as indicated on an individual basis.
Qualifications:
Ability to manage Patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention and family intervention.
Ability to work well with an interdisciplinary team.
Excellent organizational skills.
Strong computer skills.
Demonstrate problem solving and decision-making abilities.
Ability to develop and maintain rapport with the Patient and family.
Excellent communication skills, i.e. listening, oral and written skills while conversing with patients, families, health professionals and community agencies.
Capacity to function independently with licensed scope of practice.
Must have the ability to handle stressful emergency situations calmly and effectively.
Minimum of 2 years of hospice experience preferred.
