For more information about ProMedica senior care services, please visit www.promedicaseniorcare.org.   Specific Job Detail Exciting opportunity to grow and lead a regional Admissions Team in Buffalo Grove!   Sign-on Bonus None Specified  Description ProMedica Senior Care is a leading provider of home health care, hospice care, skilled nursing, memory care and post-acute care.   This is an exciting opportunity for an experienced professional to lead the development of a dedicated palliative care, home health and hospice admissions team in Buffalo Grove, IL as part of our growth initiative!  The Admissions Director (AD) is a key regional leadership position responsible for coordinating the referral to admission process for palliative care, home health and hospice services.  The AD leads the admission team and provides oversight to the intake process to ensure that eligible patients receive care in a timely manner, including obtaining clinical record information, payer verification, and scheduling informational visits/starts of care.   The AD coordinates with sales, clinical, operations and regional leadership to achieve compliant growth and is a key contributor to the development of referral sources and building trusting relationships with patients and their families.   We prefer 2 years experience in home health, hospice or palliative care and 2 years experience in call center/admissions leadership and management.  Reporting experience via data and analytics required.   In return for your expertise, you'll enjoy competitive pay, excellent training, industry-leading benefits and unlimited opportunities to learn and grow.  Be a part of the team leading the nation in healthcare!   Educational Requirements RN Preferred.
