Description

The purpose of the Director of Case Management is to guide the development, implementation and continued improvement of multiple programs within the case management department, including but not limited to the Ryan White program. Additionally, this Director is tasked with overseeing a staff of case managers who are responsible for patient care. 

The Case Management Director plans, designs, develops, implements, and evaluates the case management program and the Ryan White program, as well as other specialty programs as needed, to ensure alignment with HOPES’s vision and mission and fulfill all grant requirements. Major duties include overseeing staff who consist of case managers and case management service coordinators who are responsible for direct patient care, ensuring optimal daily operations of the case management department, overseeing the Ryan White program, and collaborating with a multidisciplinary clinical and administrative team to align with the vision and mission of HOPES.

Position Requirements

* Due to real and potential conflicts of interest that exist when HOPES Behavioral Health providers render services to their coworkers, it is HOPES’ policy (Policy HR0005) that HOPES employees may not access HOPES Behavioral Health Services. For the same reason, current Behavioral Health patients/clients cannot be employed by HOPES. Accordingly, you hereby acknowledge and understand that if you are receiving services from a HOPES Behavioral Health provider at the time you are offered and accept employment with HOPES, you have the option of rescinding your acceptance of employment or finding a new behavioral health provider outside of HOPES before starting employment with HOPES.

 

  • Bachelor or Master’s degree in Social Work or Nursing,
    OR closely related field OR equivalent combination of education and experience is required.
  • Significant experience in leadership and patient care within a healthcare setting required.
  • Knowledgeable in data analysis and reporting required.
  • Significant experience in multi-level staff supervision required.
  • Demonstrated skill in communication and problem resolution required.
  • General business acumen required.
  • Valid BLS required prior to hire, per HRSA compliance
  • Experience working with HIV-infected individuals, those with substance abuse and/or mental health issues and those experiencing homelessness preferred
  • Bilingual in Spanish & English preferred.
 
About the Organization Northern Nevada HOPES is a nonprofit community health center operating two clinics and a transitional housing community in the heart of Reno, Nevada. Our one-stop-shop integrated care model helps us meet our patients where they're at and make medical and behavioral health care more accessible to medically under-resourced populations.

Services we offer include adult and pediatric primary care, chronic disease management, nutrition services, behavioral health counseling, substance use counseling and treatment, case management, on-site pharmacies, and more! We accept most insurance plans, including Medicare and Medicaid, and offer discounted services for uninsured patients, so anyone who needs care can access our services.
