Full Time Hours Available: 8:00AM-4:30pm, Monday-Friday

  • Master the details: Accurately enter and manage patient data, ensuring every referral, admission document, physician order, and CTI is complete, signed, dated, and uploaded flawlessly into the HCHB system.
  • Stay ahead of the curve: Monitor DME orders, medical supplies, physician signatures, and level-of-care billing accuracy with eagle-eyed precision.
  • Be the communication hub: Handle incoming calls with warmth and efficiency, route messages accurately, and keep the entire team connected.
  • Support the big picture: Prepare IDT agendas, update plans of care, mail/fax UPOCs, and create organized facility binders-because every detail matters.

Position Requirements

What We Need from You:

  • Uncompromising attention to detail – you spot errors before they happen and take pride in flawless documentation.
  • At least 1 year of healthcare managed care coordination experience (hospice or similar strongly preferred).
  • Proficiency with EMR/HCHB systems and a knack for learning new technology quickly.
  • Exceptional communication, negotiation, and relationship-building skills.
  • Ability to work autonomously while collaborating with a passionate interdisciplinary team.
  • A professional, assertive, and flexible mindset that thrives under pressure.
