Description

Program Description:

Catholic Charities of New Jersey has been awarded a grant to work as a consortium to implement the 2013 Disaster Case Management Services project of the New Jersey Department of Human Services, Division of Family Development, for families impacted by Hurricane Sandy. This federally funded program will run from April 2013 through October 2014 to provide time-limited case management to survivors of Hurricane Sandy.

Catholic Charities, Diocese of Trenton is the Managing Agency, and had partnered with the other four Catholic Charities of the Dioceses of Camden, Metuchen, Paterson and the Archdiocese of Newark.  Consortium members collectively serve residents of all 21 counties in New Jersey.

 

POSITION:   Case Manager

JOB SUMMARY:   Responsible for direct case management services to disaster survivors and their families through advocacy, information and referral, crisis intervention and recovery services. 

 ESSENTIAL FUNCTIONS:

  1. Assesses client needs to address identified disaster related un-met needs.
  2. Develops, implements and monitors recovery plans to return disaster survivors to a pre-disaster level of well-being.
  3. Provides crisis intervention to individuals and families facing emergencies around food, shelter, medical, re-construction and other needs as present by client.
  4. Utilizes knowledge of the resources available at all levels of government as well as community resources.
  5. Advocates and interacts with other service providers on behalf of participants.
  6. Maintains client records; ensures all data is current in CAN database; completes statistical reports.
  7. Attends Agency's in-service training and unit meetings; successfully completes required FEMA trainings and certifications.
 
Position Requirements

MINIMUM QUALIFICATIONS: 

High School diploma or GED +3 years of relevant case management experience in the social services field; OR Bachelor’s degree in human services field.   Related experience strongly preferred.

LICENSES/CERTIFICATIONS NEEDED:  Valid drivers' license in good standing.

MANDATED PROGRAM REQUIREMENTS:

- Candidate must have own reliable transportation.   Most case manager positions will be mobile.   We offer mileage reimbursement allowance.

- Position requires strong typing / data entry, WORD, Excel, and writing skills.   Must be able to pass skills testing.

- DRCC or other crisis response credential / training a plus.

Please note that these hourly positions are 40-hrs a week and will run through October 2014, when the program funding will terminate.

Proposed Salary $25 - $30/hr.  
Schedule 9am to 6pm OR 10am to 7pm; and some Saturdays
