All qualified applicants will receive consideration for employment without regard to race, color, religion, sex, national origin, disability status, protected veteran status or any other characteristic protected by law.   Description Supervised Independent Living Case Manager - Full-Time | Temp-to-Hire | Philadelphia County Hybrid schedule with three days in the office and two remote days, Monday through Friday, 9:00 a.m. to 5:00 p.m., with flexibility based on program needs. Provide case management services to older adolescents participating in a Supervised Independent Living program. Develop and implement individualized service plans focused on life skills development, independent living, and successful transition to adulthood. Coordinate community resources, secure housing supports, advocate for youth, facilitate life skills groups, provide crisis intervention, complete assessments, develop discharge plans, and collaborate with families, referral sources, and multidisciplinary providers. Maintain accurate documentation and ensure compliance with all program and regulatory requirements.   Position Requirements BSW and at least two years of experience working within child, youth, and family service systems required. Knowledge of Pennsylvania child welfare regulations and Philadelphia DHS standards preferred. Strong assessment, communication, documentation, organizational, and computer skills required.
