This position is part-time to cover a caseload of 5-10 patients. Hours are Monday-Friday daytime, visits may be scheduled anytime the social worker and patient/family wish. Key Responsibilities: Complete psychosocial, financial, environmental, and community needs assessments and documentation in a timely manner. Assist with advance directives, funeral planning, and grief and end-of-life counseling. Attend IDT meetings and participate in care planning as needed.   Position Requirements MSW required. Home health or hospice experience preferred. Must be comfortable making home visits. Current driver's license and reliable transportation.   This position is currently accepting applications.
