We are seeking a Social Worker to provide psychosocial support and services to residents in a continuing care retirement community under the supervision of the Director of Social Services. Responsibilities include assisting with discharge planning, completing MDS documentation, addressing residents' psychosocial needs, and collaborating with the interdisciplinary team.   Position Requirements BSW is required.
