Description

JOB SUMMARY: Responsible for delivering case management/counseling services for clients and families.

ESSENTIAL FUNCTIONS:

  1. Design, implement and monitos individual program plans for clients.
  2. Assess client needs; investigate their eligibility for services.
  3. Counsel and aid individuals and families requiring agency assistance.
  4. Maintain client records and complete required documentation according to agency and accreditation standards. This may include, but is not limited to data entry, progress notes, activity reports, time sheets and logs.
  5. Coordinate client activities.
  6. Assist clients with accessing other services (job training, counseling, housing, medical services, etc.)
  7. Network with other providers to generate community resources for participants.
  8. May provide supportive counseling in individual and group settings.
  9. May transport consumers to appointments, etc.

PROGRAM RESPONSIBILITIES:

  • Conduct PALS program intake and assist in the orientation of clients
  • Ensure standardized assessments are completed by PALS parents pre and post treatment
  • Maintain weekly contact with PALS parents
  • Research and maintain database of current case management resources
  • Facilitate biweekly case conference meetings with PALS staff
  • Coordinate client scheduling
  • Assist Program Coordinator in the day-to-day administrative running of the program
  • Assist with child care and client transportation, as needed

Position Requirements

MINIMUM QUALIFICATIONS:  Bachelor’s degree in human services field, social work, psychology, sociology, guidance and counseling, child development, criminal justice, education or related discipline; OR High School Diploma/GED + 5 years of human services experience; OR relevant professional credential such as CPRP, CRC, CADC + 2 years related experience, OR a combination of one or more years of college + related work or life experience together equaling four years; OR currently being in the last semester of college working towards a Bachelor’s degree in the human services field.  40-hour domestic violence training required.

LICENSES/CERTIFICATIONS NEEDED:  Valid driver’s license required.

MANDATED PROGRAM REQUIREMENTS:  Providence House is a 24-hour residential program, as such requires staffing at all times.  When accepting a position with this agency it is important to know that the regular shift at the time of hiring is subject to change during the course of your employment, and that at times of staff emergencies or special events you may be required to fill shifts other than (instead of) your regular shift. This may include holidays.
