The physician will provide comprehensive, patient-centered palliative care services to patients with serious, chronic, and life-limiting illnesses. This role focuses on improving quality of life by addressing physical symptoms, emotional well-being, psychosocial needs, and goals of care while working collaboratively with patients, families, caregivers, and healthcare providers.   Key Responsibilities Provide comprehensive palliative care consultations and ongoing management in outpatient, inpatient, skilled nursing, and/or home-based settings as assigned. Evaluate, diagnose, and manage pain and other complex symptoms associated with serious illness. Conduct goals-of-care discussions, advance care planning, and end-of-life care conversations with patients and families. Collaborate with primary care physicians, specialists, nurses, social workers, case managers, and other interdisciplinary team members to develop individualized care plans. Coordinate transitions of care across healthcare settings. Educate patients, families, and caregivers regarding disease progression, treatment options, and symptom management. Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR). Participate in quality improvement initiatives and interdisciplinary team meetings. Provide consultation and education to healthcare staff regarding palliative care best practices. Ensure compliance with all federal, state, and organizational policies and regulations   Category Provider   Exempt/Non-Exempt Exempt   Location Suffolk County   Full-Time/Part-Time Full-Time   Position Requirements Qualifications Required: Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO). Board Certified or Board Eligible in Hospice and Palliative Medicine. Completion of an accredited residency program in Internal Medicine, Family Medicine, or another applicable specialty. Current New York State medical license or eligibility for licensure. DEA registration and New York State controlled substance certification. Excellent communication, interpersonal, and clinical decision-making skills. Commitment to compassionate, patient-centered care. Preferred: Fellowship training in Hospice and Palliative Medicine. Experience in outpatient and community-based palliative care. Experience working with interdisciplinary healthcare teams. Familiarity with EPIC or other electronic medical record systems. Bilingual abilities are a plus. Physical Requirements Ability to travel between practice locations as needed. Ability to perform patient examinations and clinical procedures. Ability to sit, stand, walk, and use standard medical equipment throughout the workday.
