Children's Mobile Intervention Mental Health Case Manager needed for full-time, temporary postion in Philadephia.
The Children's Mobile Intervention Mental Health Case Manager partners with the Intervention Specialist to support youth in crisis and their families in home, school, and community settings. Responsibilities include providing crisis stabilization, case management, behavioral interventions, caregiver support, assessments, referrals, treatment planning, documentation, and coordination with behavioral health, education, child welfare, and juvenile justice systems. The role assists families in accessing services and community resources, monitors treatment progress, and participates in discharge planning and interdisciplinary collaboration.

BSW required. Must have at least two years of experience providing behavioral health interventions or case management services to children and youth, with training or experience in crisis intervention, de-escalation, and crisis management. |
