Provides clinical social work services across inpatient and outpatient settings, including case management for glaucoma research programs. Conducts psychosocial assessments, supports high-risk patients, develops discharge plans, addresses barriers to care, and coordinates community resources. Collaborates with interdisciplinary teams, documents in EMR/REDCAP, tracks research activity, and supports projects.   Position Requirements PA LSW; strong communication, organization, and case management skills.
