The Case Manager provides screening, assessment, advocacy, and ongoing case management services for homeless families placed in shelter who are transitioning back into the community. The Case Manager conducts assessments, facilitates goal-setting discussions, develops service plans, and provides follow-up support. Responsibilities also include offering advocacy, modeling appropriate behaviors, and providing tools necessary to help participants overcome barriers. The Case Manager is responsible for maintaining detailed documentation of all interactions, including referrals, outcomes, and next steps. The role includes assisting participants in establishing and working toward short-term goals, with regular follow-up to ensure progress in daily activities that support stabilization and movement toward securing stable housing. As participants gain independence, the frequency of check-ins will be adjusted to meet their evolving needs.   Position Requirements BSW with strong communication and organizational skills, basic computer knowledge, and the ability to work independently in a fast-paced environment.   This position is currently accepting applications.
