The Social Services Director is responsible for overseeing and administering the facility’s social services program, supervising social work staff, and collaborating with interdisciplinary teams to support residents’ psychosocial, emotional, and discharge planning needs. This role provides counseling and support to residents and families, conducts social service assessments, develops and updates care plans, maintains documentation, and coordinates discharge planning and community resource referrals. The Director serves as a liaison between residents, families, outside agencies, and facility leadership to ensure resident rights and quality of care are maintained. Additional responsibilities include coordinating care plan meetings, assisting with admissions coverage as needed, completing administrative reports, ensuring compliance with all applicable regulations and safety standards, and promoting a compassionate, resident-centered environment.   Position Requirements NJ CSW or NJ LSW, strong interpersonal, written, verbal, and problem-solving skills, and knowledge of governmental regulations related to social services in long-term care settings.     This position is currently accepting applications.
