Provides psychiatric rehabilitation services in facility and community settings, helping individuals build and maintain skills for independent living, employment, and socialization. Facilitates groups, supports individualized goals, and ensures access to needed services while promoting community integration. Maintains a caseload, tracks progress and contributes to rehabilitation plans while meeting productivity standards. Collaborates with providers, addresses service gaps, and supports crisis intervention as needed. Participates in training, meetings, and program support, including reports and coverage.   Position Requirements BSW with two years of direct mental health experience, including one year in psychiatric rehabilitation services. Strong communication, documentation, and computer skills are essential, along with the ability to work independently and as part of a team.
