This role will be 8-10 weeks, starting at the end of June or early July. Responsibilities include: Discharge planning with rehab residents, admissions, interdisciplinary care planning. Evaluation and assessment of resident social service needs and concerns. Completion of social service documentation including admission paperwork, activity programs, progress notes, and care plan meetings. Addressing resident concerns in a timely manner.
