Responsibilities include completing initial psychosocial assessments and supporting patient admissions; developing and implementing care plans in collaboration with the interdisciplinary team; monitoring and reporting changes in patients' social and emotional status; consulting with physicians regarding care plan adjustments; and maintaining timely, accurate clinical documentation. The role supports patients and families with coping, resource coordination, and discharge planning, while also coordinating visit schedules and participating in team meetings and in-service trainings. Additional responsibilities include providing monthly supervision to social work assistants and contributing to overall care team effectiveness by addressing psychosocial factors impacting health outcomes.   Position Requirements MSW, with two or more years of experience preferred, including at least one year in a healthcare setting.
