The Social Worker/Admissions Coordinator manages admissions, care and discharge planning, and interdisciplinary coordination while providing counseling to residents and families. Responsibilities include overseeing documentation, maintaining census and referral relationships, ensuring regulatory compliance, supporting staff development, and contributing to departmental operations, budgeting, and training while maintaining accurate, confidential records.   Position Requirements BSW with knowledge of geriatric populations, strong assessment and problem-solving abilities, and excellent communication skills.
