Provide psychosocial support to residents, addressing mental, emotional, and social needs. Conduct assessments using tools such as MDS, PHQ-9, and social histories, develop care plans, and collaborate with the interdisciplinary team. Ensure residents' rights are protected, assist with discharge planning, and maintain accurate documentation.   Position Requirements BSW or MSW requested.
