Case Manager needed for full-time, temporary position in South Philadelphia.
Responsibilities include completing intake assessments, developing and monitoring individualized service plans, maintaining case files, and connecting residents to medical, mental health, housing, and social services. The Case Manager assists residents with appointments, housing applications, and transportation as needed, and documents progress, outcomes, and housing activities.
The role involves collaboration with the Program Director/Supervisor to approve admissions, facilitate volunteer-led life skills groups, schedule resident responsibilities, and conduct regular check-ins, including follow-up with former residents. Staff participate in training, flexible schedules, evening and weekend activities, and monthly on-call rotations.

BSW with at least two years of social service experience and demonstrate empathy for individuals experiencing homelessness or living with HIV/AIDS. Knowledge of addiction, recovery, mental health, and relapse prevention is essential, along with strong organizational, communication, and computer skills, and an understanding of professional boundaries. |
