Responsibilities: Restore patient mobility, function, and independence through individualized treatment plans. Assess and evaluate residents' conditions, interpreting test results and progress to adjust therapy plans. Develop and implement personalized Plans of Treatment, including short- and long-term goals. Administer therapeutic exercises, manual therapy, and use of appropriate equipment. Evaluate the fit and use of prosthetic and orthotic devices and recommend modifications. Communicate progress and care plans with residents, families, caregivers, and healthcare team members. Ensure accurate and timely documentation of therapy services.   Position Requirements Qualifications: Graduate of an accredited Physical Therapy program. Current state licensure as a Physical Therapist. CPR certification. 2–3 years of related experience preferred (or equivalent combination of education/experience).
