Description

Skilled Nursing Home Social Worker needed for part-time (3 days/week), temporary (12 week) position in Montgomery County.

The Social Worker/Admissions Coordinator oversees resident admissions, care planning, discharge planning, and interdisciplinary coordination while providing supportive counseling to residents and families. This role manages admission and documentation processes, maintains census and referral relationships, ensures regulatory compliance, supports staff development, and promotes positive resident and employee relations. The position also contributes to budgeting, training, and overall departmental operations while ensuring timely, accurate, and confidential records.

 
Position Requirements

BSW or MSW with working knowledge of the geriatric population, including psychological needs and issues, physical needs and issues, and local resources. Strong communication and interpersonal skills for daily interactions with residents, family members, peers, and external constituencies. Ability to make independent judgments and demonstrate initiative. Excellent assessment and problem-solving skills. Excellent oral and written communication skills.
