Description

Case Manager needed for full-time, temp to hire position in Philadelphia.

The Case Manager develops and coordinates comprehensive, goal-oriented plans of care, including psychosocial assessment, discharge planning, and collaboration with interdisciplinary teams. This role serves as a liaison among patients, families, utilization management, and hospital staff while managing an assigned caseload and supporting departmental operations.

The Case Manager promotes effective communication, professionalism, patient satisfaction, confidentiality, and continuous improvement, while supporting safety initiatives and maintaining required competencies. Responsibilities include leading care planning and discharge processes, conducting assessments, documenting electronic records, coordinating family conferences, arranging post-discharge services and community resources, and ensuring compliance with CMS and Medicare guidelines. The role also supports cost-effective resource use, staff education, and utilization management as needed.

 
Position Requirements

PA LSW or LCSW licensure, three to five years of relevant case management experience, preferred utilization review and/or CCM certification, and strong communication and organizational skills.
