The Social Worker supports the delivery of social services programs by assessing residents' psychosocial needs, developing and maintaining individualized care plans, and collaborating with interdisciplinary teams. This role conducts social histories, provides referrals to community resources, assists with transportation, financial assistance, and Medicare and Medicaid applications, addresses billing concerns, and maintains accurate documentation. The Social Worker advocates for resident rights, maintains confidentiality, educates residents and families on advance directives, addresses grievances, participates in council meetings, and promotes ethical standards of care.   Position Requirements A BSW with healthcare, long-term care, or residential experience is preferred. Strong knowledge of regulations, excellent communication and documentation skills, sound judgment, and the ability to manage multiple priorities in a fast-paced environment are required.   This position is currently accepting applications.
