The Position

The family resource navigator supports the work of the Access and Flow team by assisting families in finding and utilizing resources available to them.

This service supports Holland Bloorview inpatient parents/caregivers and clients who require help to apply for resources that can address their healthcare-related costs, as well as other specific needs.

The resource navigator provides assistance with form completion and the gathering of documents in order to apply for several different types of resources. Applications can be a long, hard and intimidating process for families; this role helps minimize this stress by supporting inpatient clients and families, allowing them to focus on planning with their care teams and their own well-being. The resource navigator will have the ability to connect with clients and families using a strength-based and solution-focused approach and to recognize their unique needs.

This role will report to the Manager, Client Access and Flow.

Requirements:

  • Bachelor’s degree in a social service (Social Work preferred) or healthcare related field (the following may substitute: associate’s degree in a social service or healthcare related field with an additional two (2) years of directly related experience.
  • Experience in healthcare setting (preferred)
  • Ability to work effectively/collaborate with many different disciplines/co-workers
  • Ability to have a flexible work schedule including some evenings
  • Experience in completing government funding application: Assistance for Children with Severe Disabilities (ACSD), Special Services at Home (SSAH), Ontario Disability Support Program (ODSP), etc.
  • Knowledge and experience in accessing other types of resources: Easter Seals, Wheel-Trans applications, charitable foundations (Jennifer Ashleigh, etc).
  • Ability to anticipate next steps, be proactive and collaborate with team members
  • Ability to use integrated technology platforms and virtual care coordination tools
  • Ability to use good judgment and problem-solving skills, able to effectively respond to difficult situations and resolve conflicts
  • Ability to work independently in a fast-paced environment
  • Excellent written and verbal communication skills
  • Ability to organize and prioritize workload; effective time and task management skills in achieving program initiatives and priorities
  • Ability to successfully complete tasks with little or no supervision.
