The Pediatric Social Worker provides assessments, care planning, interventions, and follow-up for patients and families across the continuum of care. This role includes documenting services in compliance with regulatory and facility standards, coordinating resources and services to support patient outcomes, and integrating care with the healthcare team. The social worker advocates for patient safety, rights, and well-being, collaborates with families, caregivers, and providers, and arranges services with community agencies and medical partners. Additional duties may be assigned.   Position Requirements PA LSW with two years of experience, is preferred. Experience conducting and documenting psychosocial assessments and interventions addressing physical, functional, social, emotional, and educational needs is required. Healthcare experience in a pediatric setting is preferred, including resource coordination, understanding payment systems, addressing psychosocial barriers to care, and working with custody and guardianship matters.   This position is currently accepting applications.
