This position offers the opportunity to make a meaningful impact with medically complex patients while working within a collaborative care team. LTACH Social Worker - FT | Temp | Philadelphia The Social Worker supports patients with complex medical needs by providing psychosocial care, coordinating services, and leading discharge planning. Key Responsibilities Conduct psychosocial assessments, develop care plans, and provide counseling to patients and families Lead discharge planning and coordinate transitions to home health or skilled nursing facilities Serve as a liaison among patients, families, providers, and community resources Assist with benefits, insurance navigation, and service referrals Maintain accurate clinical documentation Provide crisis intervention and behavioral support   Position Requirements Qualifications MSW, LSW preferred Preferred experience in long-term care, med-surg, or rehabilitation settings Strong communication, problem-solving, and teamwork skills   This position is currently accepting applications.
