The Crisis Intake and Primary Care Navigator at Carlington Community Health Centre plays a vital role in delivering integrated, client-centered care to individuals and families in the Carlington community. Program-Related Activities

  • Crisis Intake & Client Assessment : The role involves being the first point of contact for Carlington residents in crisis, providing immediate support, risk assessments, safety planning, and advocacy. It includes conducting psychosocial assessments to identify client needs based on social determinants of health such as housing, income, and food security. Short-term case management plans are developed to address psychosocial barriers, while also advocating for clients at both individual and systemic levels. Crisis intervention services are offered both in person and over the phone. The role collaborates with harm-reduction workers to promote support services and assists with the Emergency Food Cupboard in partnership with the Ottawa Foodbank Link2Feed program. Additionally, the position oversees internal counseling referrals and waitlists with the counselor team and supports CCHC’s health promotion initiatives.
  • Integrated Care Navigation: This role supports clients in accessing healthcare, mental health, addiction services, and social supports while addressing barriers such as financial challenges, transportation, and cultural considerations. It facilitates smooth transitions between service providers to ensure continuity of care and guides clients through Ontario’s healthcare and social service systems, including OHIP, ODSP, OW, and housing assistance. The position involves documentation, referrals, and collaboration with community service providers to build client support networks. Additionally, it contributes to program development through social prescribing, tracks service utilization, ensures confidentiality, and participates in volunteer and student supervision. Reporting on client outcomes and program effectiveness supports continuous quality improvement.
 
Position Requirements

Education

  • Bachelor’s degree in social work (BSW) from a recognized university in a relevant discipline or equivalent combination of some post-secondary education and experience in counselling.
  • Registration and good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
  • Master’s level in social work (MSW) an asset

Professional Experience

  • Minimum of three years of experience in community health, crisis intervention, or working with populations impacted by the social determinants of health, including food insecurity and substance use.
  • Experience in providing community-based mental health and counselling services including crisis intervention, suicide risk assessment and intervention, short-term supportive counselling, harm reduction, group work, case management, referral, and advocacy.
  • Experience in program development, implementation, monitoring, and evaluation.
  • Experience in a non-profit, community-based organization.

Key Competencies

  • Thorough knowledge of and proficiency in current assessment, intervention and counselling techniques.
  • Thorough knowledge and understanding of community resources; relevant city service- provider networks, community-based health care, and income-maintenance legislation and services, and services relevant to addressing gender-based violence, and mental illness. Knowledge of the Carlington client-service area an asset.
  • Sound knowledge of social determinants of health and their impact on healthcare access.
  • Excellent knowledge of and experience with a harm reduction approach.
  • Experience working with underserved and marginalized groups
  • Excellent interpersonal and communication skills, oral and written.
  • Excellent time management and organization skills.
  • Demonstrated ability to work effectively in a crisis situation.
  • Demonstrated ability to work in an interdisciplinary team environment.
  • Demonstrated proficiency in the use of computers and various software applications, including Microsoft Office programs, client database software, and electronic medical record keeping in a client service setting.
