Outpatient & Home Care Focus | Continuum of Care (COC)
Looking for flexible weekday hours and work that allows you to build real relationships with your patients? Community Physical Therapy (CPT) is seeking a PRN Physical Therapist to support our Continuum of Care (COC) program, delivering outpatient and home care therapy services to residents living in Alden Assisted and Independent Living communities.
This role is ideal for a therapist who enjoys the variety of outpatient treatment paired with home care visits, all within a familiar campus environment.
Why You'll Love This Role
You'll enjoy flexible weekday scheduling, Monday through Friday between 7:00 a.m. and 6:00 p.m., with no weekend requirements. Care is provided in outpatient clinics using TheraOffice and in the home care setting using HCHB. You'll work closely with Family Home Health and an interdisciplinary team to deliver consistent, high-quality care-right where patients live. As part of a therapist-led organization, you'll be supported by leaders who understand your work and value clinical excellence.
What We're Looking For
A licensed Physical Therapist with availability during weekday hours and a strong commitment to patient-centered care. Experience in outpatient and/or home care settings is preferred, but we are happy to provide training.
