The Temporary Full-Time Care Manager provides short-term coverage for a caseload of 10-15 clients in Delaware County, coordinating care across benefits, housing, health, finance, social, and vocational needs. The role includes in-person visits, advocacy, coaching, crisis support, budgeting assistance, and help with benefits applications, while also supporting the Financial Services team with scheduling and follow-up. Travel for client visits typically ranges from 20 to 45 minutes. The position requires consistent documentation, including weekly notes, accurate time tracking, monthly billable submissions, timely receipt submission, and completion of assessments and goal plans.   Position Requirements BSW required with experience in social services or care management, strong organizational and communication skills, knowledge of community systems, and comfort with travel and in-person client work.   This position is currently accepting applications.
