Position Summary

The MDS Coordinator LVN conducts and coordinates MDS assessments and care plans in compliance with federal, state, and local regulations. This role supports the Interdisciplinary Care Plan Team by ensuring documentation is timely, accurate, and reflective of each resident’s needs.

 

Key Responsibilities

  • Complete and coordinate required MDS assessments including care area triggers and care plan updates.
  • Evaluate resident conditions and determine required assessment components.
  • Assist in facilitating care plan meetings, including communication with families.
  • Ensure compliance with all required timelines for admission, comprehensive, quarterly, annual, and significant-change assessments.
  • Support nursing and interdisciplinary staff in understanding and documenting information required for MDS accuracy.
  • Prepare and transmit MDS assessments to appropriate agencies on time.
  • Participate in surveys, in-services, and other required training sessions.

 

Qualifications

  • Nursing degree from an accredited college or university.
  • Active, unencumbered Texas LVN license.
  • Minimum 1 year of experience in long-term care or a related healthcare setting.
  • Strong clinical assessment, organization, and communication skills.
  • Knowledge of long-term care regulations, documentation standards, and MDS processes.
  • Ability to work professionally with residents, staff, families, and regulatory agencies.

 

Working Conditions & Physical Requirements

  • Indoor work environment with regular interaction with residents and staff.
  • Frequent walking, sitting, bending, lifting, and chart handling.
  • Possible exposure to infectious waste and bodily fluids; must follow all safety and infection-control standards.
  • Ability to work extended hours during surveys, deadlines, or resident condition changes.
