Position Summary

The MDS Coordinator RN is responsible for conducting, coordinating, and completing the Minimum Data Set (MDS) assessments in accordance with federal, state, and local regulations. This role works closely with the Interdisciplinary Care Plan Team to ensure each resident receives an accurate, holistic, and timely assessment and care plan.

 

Key Responsibilities

  • Complete, coordinate, and manage all MDS assessments including care area triggers and interdisciplinary care plans.
  • Ensure each required assessment meets regulatory deadlines such as admission, comprehensive, quarterly, annual, and significant-change assessments.
  • Evaluate resident conditions and medical data to determine assessment needs.
  • Coordinate and lead care plan meetings, ensuring involvement and communication with families when appropriate.
  • Educate staff about documentation accuracy and the regulatory importance of MDS completion.
  • Transmit all assessments timely to state and federal agencies.
  • Participate in state and federal surveys and provide documentation as requested.
  • Assist in training staff on assessment requirements, coding, and compliance.

 

Qualifications

  • Nursing degree from an accredited college or university.
  • Active, unencumbered Texas RN license.
  • At least 1 year of experience in long-term care or related healthcare setting.
  • Knowledge of long-term care regulations, nursing practices, and assessment procedures.
  • Strong clinical assessment, communication, and leadership skills.
  • Ability to work independently, manage deadlines, and prioritize complex tasks.

 

Working Conditions & Physical Requirements

  • Indoor work with frequent interruptions and interactions with residents, families, and staff.
  • Regular walking, sitting, bending, lifting, and charting throughout the workday.
  • Potential exposure to infectious materials; must follow PPE and infection-control policies.
  • Must be able to work extended hours during surveys or urgent situations.
