Responsibilites: To assist in the planning, implementation, and evaluation of recreational, social, intellectual, emotional, and spiritual programs, in accordance with the resident’s assessment and care plan, and as may be directed by the Director of Social Services. Key Qualifications: Bachelor’s degree in Social Work from an accredited school; a Master’s degree and active state licensure in Social Work (LSW or equivalent) preferred or as required by state regulations. Minimum of one (1) year of supervised social work experience in a healthcare or long-term care setting, providing direct services to individuals and families. Comprehensive knowledge of psychosocial assessment, care planning, and discharge planning within a long-term care environment. Demonstrated ability to address the emotional, social, and psychological needs of residents while maintaining compliance with state and federal regulations. Proven communication, problem-solving, and advocacy skills to support residents, families, and interdisciplinary team collaboration.
