The Hospice Social Worker provides emotional support, counseling, and resource coordination to patients and families coping with terminal illness. Working as part of an interdisciplinary hospice care team, the Social Worker ensures that each patient's physical, emotional, and psychosocial needs are compassionately addressed.   Position Requirements PA LSW or LCSW required. Prior Hospice, palliative care, or healthcare social work experience required. Excellent communication, counseling, and organization skills.
