Status: Full-Time or PRN
Community Physical Therapy (CPT) is a therapist-led organization serving the greater Chicagoland and southern Wisconsin areas. We're seeking a dedicated Speech-Language Pathologist to join our team at Alden Valley Ridge.
CPT provides Physical, Occupational, and Speech Therapy across skilled nursing, outpatient, assisted living, and independent living settings. Our leadership team is composed of licensed therapists who understand and value the work you do every day.

Responsibilities
• Provide speech therapy services in accordance with CPT and facility standards
• Maintain accurate and timely documentation
• Collaborate with patients, families, and interdisciplinary teams to achieve goals
Qualifications
• Master's degree in Communication Disorders
• Illinois license (or eligibility)
• CCC-SLP or actively completing CFY
Preferred: SNF experience; Net Health familiarity (training provided)
