The Social Worker is responsible for case management and care planning, which includes conducting psychosocial assessments and developing/updating individualized care plans with the interdisciplinary team. They are the main source of resident and family support, providing counseling, resolving conflicts, and managing transitions within the facility. A core function is coordinating discharge planning, arranging necessary post-discharge services, and educating families for smooth continuity of care. Finally, they ensure regulatory compliance by maintaining accurate, timely documentation, including the MDS, in the medical record.   Position Requirements NJ CSW or LSW Experience in a geriatric, LTC, or SNF setting is essential. Required knowledge includes social work ethics and practices, along with regulations such as Medicare and Medicaid.
