This is a rewarding opportunity to contribute to high-quality, client-centred care in a community-based setting focused on health equity and social determinants of health. Responsibilities include Provide high-quality, client-centered nursing care to individuals and families from diverse and marginalized communities. Conduct clinical assessments, deliver treatments, and manage follow-up care in a primary care setting. Support chronic disease management, health promotion, and illness prevention through education and clinical interventions. Collaborate with physicians, nurse practitioners, and allied health professionals to deliver coordinated, holistic care. Maintain accurate documentation and uphold professional and organizational standards of care.     Position Requirements Qualifications Current registration in good standing with the College of Nurses of Ontario (CNO) as an RN or NP-Primary Health Care.  Undergraduate degree in Nursing from a recognized university (Master’s degree required for NPs). Additional relevant training or certification in areas aligned with community health services. 3–5 years of full-time nursing experience in a community-based, primary care, or public health setting (experience in both community and acute care is an asset). Demonstrated experience working with marginalized and underserved populations, including individuals facing social and economic barriers. Strong clinical assessment, communication, and interprofessional collaboration skills. Proven ability to support inclusive, culturally safe, and trauma-informed care. Experience in program development, implementation, and evaluation is an asset. Previous work in non-profit or community-based organizations is considered an advantage. Linguistic Profile Fluency in English is required. A third language relevant to our client population is an asset. Personal Suitability Sensitivity to the needs of marginalized and diverse clientele. Commitment to health promotion concepts, strategies, and philosophy. Commitment to community-based health care and social services, and strength-based philosophy and practices.
