Description

JOB SUMMARY:  Responsible for providing outreach and delivering case management/counseling services for clients and families for Mercer County Services (MCS) - Returning Citizens to the Community Program (RCCP).    

ESSENTIAL FUNCTIONS: 

  1. Represents the program in the community and provides outreach to referring entities, community resources, vendors, and consumers.
  2. Provides initial screening including assessing client needs and investigating their eligibility for services.
  3. Designs, implements and monitors individual program plans for clients.
  4. Counsels and aids individuals and families requiring agency assistance.
  5. Maintains client records and completes required documentation according to agency and accreditation standards.  This may include, but is not limited to data entry, progress notes, activity reports, time sheets and logs.
  6. Coordinates client activities with a focus on shelter placement, stabilization, and connection to rapid rehousing resources.
  7. Assists clients with accessing other services in the community (job training, counseling, housing, medical services, etc.)
  8. Networks with other providers to provide collaborative street outreach in the field and to generate community resources for participants.
  9. Communicates regularly with CEASe center, RRH Program Director and other program staff about outreach activities.
  10. May transport consumers to appointments, etc.

 

 

 
Position Requirements

MINIMUM QUALIFICATIONS:

  • High School / GED + 5 years of human services experience; OR
  • Relevant professional credential such as CPRP, CRC, CADC + 2 years related exp., OR
  • Combination of one or more years of college + related work or life experience together equaling four years; OR
  • Bachelor’s degree in human services field, rehabilitation, nursing, or a related discipline.

LICENSES/CERTIFICATIONS NEEDED: Valid driver’s license

PREFERRED SKILLS:

  • Bilingual Spanish preferred.
  • Lived experience with housing insecurity
  • Skills to include experience with community outreach, case management, database entry and case file management experience

Proposed Salary $40,040/annually ($22/hr)  
Schedule Monday to Friday 9:00AM - 5:00PM
