Dynamic and results-driven to oversee the development, implementation, and quality of training programs. This role demands a forward-thinking leader with excellent critical thinking, organizational, and troubleshooting skills, capable of ensuring the success and continuous improvement of training initiatives. The ideal candidate will be responsible for managing budgets, ensuring high-quality programs, and driving the overall vision and strategy of training functions, while providing hands-on oversight and evaluations. The ability to adapt and solve problems quickly while maintaining an optimistic outlook and strong leadership is essential.

Care Management Manager

Reports to:

Associate Vice President

Position Description:

Drive departmental growth through strategic planning, employee development, and process optimization. Oversee budgeting and lead initiatives that generate revenue, maximize referrals, and enhance service delivery. Ensure operational excellence and strengthen partnerships through effective leadership and execution.

Essential Responsibilities and Duties:

  1. Leads strategic initiatives to drive departmental growth and performance, aligning with organizational goals and objectives.
  2. Collaborates with cross-functional teams to identify new revenue streams and optimize existing ones, driving sustainable growth and profitability.
  3. Maintains a comprehensive understanding of current contracted payers, both federal and non-federal, and acts as a key liaison to the payer relations team, supporting payer strategies.
  4. Manages departmental budgeting, including planning, forecasting, and monitoring expenditures to ensure alignment with financial objectives.
  5. Ensures team members are trained to apply critical thinking in navigating complex payer scenarios to deliver unparalleled customer service and achieve the most advantageous outcomes for referrals.
  6. Participates in the Certified Training Assistant (CTA) program as a Certified Trainer, and successfully completes available leadership development programs to actively support Training & Development.
  7. Monitors key performance indicators and metrics to track progress and identify areas for improvement; implements corrective actions as needed; stays abreast of industry trends and best practices, applying relevant insights to drive strategic decision-making and innovation.
  8. Partners with key stakeholders across departments, business units, and sister companies to align initiatives, drive shared goals, and enhance organizational collaboration.
  9. Fosters a culture of excellence and accountability by providing effective leadership to direct reports and coaching managers to develop these values within their teams.
  10. Oversees scheduling, disciplinary actions, performance evaluations, and paid time off for assigned team members.
  11. Coordinates and drives monthly or quarterly gatherings, including in-services, educational sessions, and training activities, to address and resolve knowledge gaps or issues related to health plan protocols, procedures, processes, and the overall effectiveness of referral sources.
  12. Maintains an excellent attendance record and drives excellent attendance of departmental team members through accountability measures and incentives.
  13. Enhances organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
  14. Adheres to all company policies and procedures regarding employment, safety and compliance and reports any concerns of non-compliance in any area to your manager immediately.
  15. Performs other related duties as requested and assigned by Executive Leadership.

Education/Experience/Knowledge Requirements:

  • Bachelor's degree and 2 years of experience in a management/supervisory position, preferred OR any similar combination of education and experience.
  • 4 years of healthcare experience in a medical office setting, preferred.
  • A deep understanding of HME rules and regulations, preferred.
  • Proven ability to build and leverage cross-functional relationships to drive collaboration, streamline workflows, and achieve strategic objectives.
  • Advanced verbal and written communication skills.
  • Strong attention to detail in composing, typing, and proofing materials, establishing priorities, and meeting deadlines.
  • Proficiency in technical tools including Microsoft Excel, Word, Teams, and Outlook.
  • Exceptional ability to manage multiple complex priorities simultaneously in a dynamic, fast-paced environment while maintaining accuracy and meeting critical deadlines.
  • Demonstrated ability to engage and collaborate effectively with stakeholders at all organizational levels, fostering professional relationships that support business objectives and drive results.

Physical Requirements:

  • In an 8 hour day the employee lifts/carries 10 LBS rarely.
  • In an 8 hour day the employee stands 1 hour.
  • In an 8 hour day the employee sits 7 hours.
  • In an 8 hour day the employee drives a vehicle 0 hours. Vehicle operation is rare, less than monthly.

Travel Requirements:

Percentage of travel expected to be 5% or less on an annual basis
