The Long-Term Care Social Worker will conduct psychosocial assessments, develop care plans, and provide counseling to residents and families to support adjustment and well-being. They assist with admissions and discharges, ensure proper documentation, participate in care plan meetings, and serve as a liaison between residents, families, staff, and community resources. The social worker advocates resident rights, addresses grievances, and ensures compliance with all applicable state and federal regulations while maintaining timely and accurate records.   Position Requirements NJ CSW or NJ LSW with long-term care experience.
