Individual and Family Case Manager needed for full-time position in South Jersey.
40 hours a week, at least 2 late days (12 pm-8 p.m. or 1-9 pm)
Provides direct counseling services and fulfills program-specific responsibilities, including life skills assessment, training, and case management for youth in the programs. Designs and updates individualized treatment plans and delivers both individual and group counseling. Develops and implements a rotating schedule of life skills instruction with corresponding lesson plans and evaluation tools. Oversee treatment plans, support advocacy efforts, and participate in crisis intervention and youth supervision as needed. Maintains required documentation, meets regularly with supervisors, and engages in professional development. Participates in agency initiatives and contributes to a positive, mission-driven work environment.
