Older Adults Case Manager, St. Louis County

Description

This full-time position located in St. Louis County supports older adults in maintaining independence, safety, and social connection while living at home. This role involves conducting assessments, creating care plans, arranging services like in-home support and transportation, and connecting clients to internal and community resources. Over 50% of the time will be spent meeting clients in the community. This role is M-F from 8:30-5:15. This position is available on a temp, temp to perm, or permanent basis.

Key Responsibilities:

  • Conduct timely intake assessments and create individualized care plans

  • Refer and connect clients to supportive services

  • Coordinate medical appointments and follow up on service quality

  • Maintain detailed records

  • Reassess clients at least every 90 days

  • Provide outreach and community education

 
Position Requirements
  • MSW, OR a BSW with a master's degree in a related field

  • Access to personal transportation
