Site Coordinators/Case Managers needed for full-time, temp to hire position in Philadelphia.
This full-time, year-round (35 hours/week) position coordinates program services and agency requirements to support pregnant and parenting students within the ELECT division. The Site Coordinator/Case Manager leads recruitment, outreach, enrollment, and provides individualized, bi-weekly case management, including at least quarterly home visits and a minimum of four service hours per participant each month.
Responsibilities include tracking student attendance and academic progress, connecting families to childcare and social services, and maintaining timely case documentation. The role involves building referral relationships, navigating city systems such as DHS, Family Court, WIC, and Truancy Court, and addressing parenting, communication, and life skills needs.
