The Social Worker provides support and counseling to patients and families facing financial, social, and emotional challenges that affect home health care. They help address personal and environmental difficulties, coordinate discharge plans, and support patient care goals. Responsibilities include assessing social/emotional factors, counseling families, collaborating with the home health team, and connecting patients with community resources. The role involves developing care plans within 72 hours of referral, maintaining accurate records, participating in care conferences, attending in-services, assisting with staff orientation, and ensuring compliance with agency policies and home health regulations.   Position Requirements PA LSW or LCSW required and at least one year of social work experience in a healthcare setting (preferably home health), valid driver’s license, and insured, reliable transportation.   This position is currently accepting applications.
