Long Term Care Social Worker needed for full-time, temp to hire position in Salem County.
Manages social work services from admission through discharge, including care transitions, discharge planning, room changes, and referrals. Leads interdisciplinary care plan meetings, completes MDS assessments, admission and psychosocial evaluations, and ensures timely, accurate documentation per State, Federal, and facility guidelines. Provides psychosocial interventions, palliative and hospice referrals, and bereavement support. Acts as Grievance Officer, educates on advanced directives, and maintains knowledge of insurance models and medical assistance applications.
